Understanding Microneedling

Microneedling is an advanced skin rejuvenation treatment that uses controlled microscopic punctures in the skin to stimulate the skin’s natural repair process, known as dermal remodelling, in which the skin lays down new collagen. At Dermatology Associates of the Bay Area, we utilize the Bellus SkinPen, the first FDA-approved microneedling device in the United States. Our PAs and board-certified dermatologists have extensive experience with microneedling treatments, both as a standalone therapy and combined with platelet-rich plasma (PRP) for enhanced results.

How Microneedling Works

Microneedling utilizes your skin’s natural wound-healing process to enhance skin texture, tone, and appearance. Ultra-fine needles create thousands of microscopic channels in the skin, which the body sees as damage requiring repair. This triggers fibroblasts to produce new collagen and elastin, and the damaged tissue is replaced with new healthy skin cells. During and immediately after the procedure, the numerous open microscopic channels also facilitate better product absorption, allowing us to tailor and enhance each patient’s treatment to their desired goal of improving acne scarring, sun damage, or pigmentary abnormalities through the use of collagen stimulators, antioxidants, or bleaching agents.

PRP Facial – The “Vampire Facial”

When microneedling on the face is combined with PRP growth factors, it stimulates even greater collagen production, resulting in a smoother and healthier skin tone. The PRP Facial, commonly known as the “Vampire Facial,” combines the benefits of microneedling with the regenerative power of your own blood platelets.

The PRP process involves collecting a small amount of your blood from your arm, similar to when you obtain routine blood work, and processing it in a specialized centrifuge to separate its components.  The platelets are isolated and concentrated into an activated platelet-rich plasma, which contains growth factors that stimulate tissue repair and cellular regeneration. This activated platelet-rich plasma is massaged into the skin immediately after microneedling, allowing the growth factors to penetrate deeply into the skin.

The Microneedling & PRp Treatment Experience

During your consultation, we analyze your skin concerns, skin type, and treatment goals, review your medical history, including any medications and conditions that may affect healing, customize the depth and technique for optimal results, and discuss realistic outcomes and timelines for improvement.

The treatment process begins with thorough skin cleansing and application of a topical numbing cream. If PRP is selected, we perform a blood draw and processing during the numbing period. We then systematically treat targeted areas with the SkinPen device. If PRP is included, fresh PRP is applied during and after microneedling. We conclude with soothing serums and detailed aftercare instructions.

Each treatment takes approximately 30-45 minutes. The treatment is well-tolerated, utilizing topical numbing and customized needle depths based on skin thickness and treatment goals. Typically, it requires a series of 3-6 treatments spaced 4-6 weeks apart for optimal results. 

Sun avoidance is extremely important following treatment and through the healing process.

Microneedling & PRp Recovery and Results

Immediately after treatment, your skin will appear red and flushed, similar to a moderate sunburn, with mild tightness and warmth in treated areas. This redness typically subsides within 24 to 48 hours, and most patients return to work within 1 to 2 days.

During recovery, expect peak redness and mild swelling for the first 2 days, followed by dry, flaking skin that begins to feel better on days 3-5. By week 1, initial healing is complete, and makeup can typically be worn. Early improvements in skin texture become visible at weeks 2-4, with continued improvement as collagen production increases through months 2-3. Optimal results are achieved over months 3-6 with multiple treatments.

Expected results include a smoother, more refined skin texture, a significant improvement in the appearance of acne scars, enhanced skin tone and luminosity, a reduction in pore size and visibility, and a diminishment of superficial wrinkles.

Is microneedling painful?

Most patients find microneedling to be very comfortable when used in conjunction with a topical numbing cream. The sensation is often described as mild vibration or light scratching. The SkinPen’s precision engineering minimizes discomfort compared to older roller-type devices.

How many treatments will I need?

Most patients benefit from a series of 3-6 treatments spaced 4-6 weeks apart. Acne scarring typically requires 4-6 sessions for optimal improvement, while general skin rejuvenation may see good results with 3-4 treatments. Our dermatologists tailor treatment plans to address your specific concerns.

What’s the difference between microneedling with and without PRP?

Microneedling alone can effectively stimulate collagen production and improve skin texture. Adding PRP enhances these benefits with growth factors that accelerate healing and promote more robust collagen synthesis. PRP is particularly beneficial for acne scarring and overall skin rejuvenation.

Can I wear makeup after microneedling?

Avoid makeup for the first 24 hours to allow initial healing. After that, mineral makeup can typically be worn, but avoid heavy foundations or products with irritating ingredients for the first week.

How long do results last?

Results vary depending on the treatment and the severity of your acne. Some patients see improvements in a few weeks, while acne scar treatments may require multiple sessions over several months for the best outcome.

Who is not a candidate for microneedling?

Patients with active acne, skin infections, open sores or wounds, certain autoimmune conditions, or those taking blood-thinning medications may not be suitable candidates for this procedure.

Is there any downtime with microneedling?

Downtime is minimal. Expect 1-2 days of redness and mild swelling. Most patients return to work within one to two days. Avoid sun exposure, intense exercise, and harsh skincare products for the first week to optimize healing and results.

What precautions should you take for Microneedling treatment?
  • Avoid excessive sun exposure for 48 hours before and for one week after treatment.
  • Stop using topical retinoids (Retin-A/Tretinoin, Differin/Adapalene, Retrinol) 24 hours before treatment.
  • Do not get treatment if you have active acne breakouts, open sores, or wounds.
  • Wait 6 months after a course of Isotretinoin (Accutane).
  • People with darker skin complexions may experience some darkening after treatment. This is usually temporary and will improve slowly with time.
  • Treatment is not recommended in the setting of active eczema, psoriasis, infections, or skin cancers in the treatment areas, uncontrolled diabetes, active autoimmune or collagen vascular disorders, immunosuppressive therapy, or Accutane treatment.  
  • Herpes cold sore prophylaxis with valacyclovir is recommended for individuals at risk.
